than others, please provide the receptionist with as many details as possible
when making your appointment. To save you time, if you are a new
patient to Hartford Orthopedic Surgeons, please go to our New
Patient & Worker Compensation Forms page, click on the appropriate
form to download it to your computer. Print it out and fill it out
completely and bring it with you the day of your scheduled appointment
